A 46-year-old Texas man, Juan Martin Flores, has been arrested and charged with healthcare fraud and aggravated identity theft for allegedly defrauding the Texas Medicaid Program of approximately $2 million.
Flores is accused of submitting over 15,000 false claims to Medicaid between 2018 and 2022 for counseling services he never provided. The indictment also alleges that he used the personal information of Medicaid beneficiaries without their consent to carry out this scheme.

If convicted, Flores faces up to 10 years in prison and a $250,000 fine for each of the 10 counts of healthcare fraud. Additionally, the three counts of aggravated identity theft carry a mandatory two-year prison sentence that must be served consecutively to any other sentence.
The investigation was conducted by the FBI, the Department of Health and Human Services – Office of Inspector General, and the Texas Attorney General’s Medicaid Fraud Control
